Making Your Own Change of Address Cards

It doesn't take much to create your own personalized change of address cards. A standard 3 x 5 card will do the trick, but for more professional results use a standard word processor such as Word Perfect®, ClarisWorks®, or Microsoft® Word to create short letters you can send to family, friends, and business associates. If you are familiar with printing address labels you can print labels for each individual and organization that sends you mail and then apply the labels to preprinted cards.

Regardless of what method you choose, make sure to include all necessary information. Below is an example of what should appear on a change of address form or card. (Although not specifically designed for that purpose, this page can be filled out in your browser and then printed.)

Front of Change of Address Card

The front of the change of address card should include a stamp (keep in mind that a postcard is significantly cheaper to mail than a letter) and the address of the business or person who regularly sends you mail:




Back of Change of Address Card

The back of the change of address card should include both your new and old address as well as your new phone number. If the organization you are notifying is a business with which you have an account, be sure to include an account number and signature. It is also important to include the date on which mail should begin to go to the new address.
